Titanium vs Gold Piercing Jewellery: Which One First?
The Short Answer
Titanium is the better choice for a new piercing. Solid gold is the better choice once a piercing has fully healed. Both are safe metals when you buy the right grade, but they solve different problems at different stages.

Why Titanium Comes First
Implant-grade titanium meets a manufacturing standard called ASTM F136, the same specification used for surgical implants. It contains no nickel, which is the metal responsible for most jewellery skin reactions. The American Academy of Dermatology lists nickel as the most common cause of allergic contact dermatitis from jewellery, so removing it from the equation matters most while a piercing is still an open wound.
Titanium is also lighter than gold. A fresh piercing is swollen and tender, and a lighter stud or ring puts less pull on the tissue while it closes. That is part of why the Association of Professional Piercers lists implant-grade titanium among its recommended materials for initial piercings.
What "implant grade" actually means
Not every piece labelled "titanium" is implant grade. Some body jewellery is made from titanium alloys mixed with other metals for cost or colour, and those alloys can still cause irritation. Look for a listing that states "ASTM F136" or "implant-grade titanium" directly. If a listing only says "titanium" with no grade, ask before you buy, or treat it as a lower grade for pricing purposes.
Why Gold Comes Second, Not First
Fourteen and eighteen karat gold are both real, durable metals for body jewellery once a piercing has healed. The catch is the alloy. Pure gold (24k) is too soft for daily wear, so 14k and 18k gold are mixed with other metals for strength. Those added metals occasionally include a trace of nickel, depending on the manufacturer, which is why gold is the second step rather than the first.
Once your piercing has fully closed and the skin barrier is intact, that small risk drops off. At that point gold has real advantages: it does not tarnish the way plated or lower-grade metals do, it holds its colour, and it is available in yellow, white, and rose tones that titanium (in its natural, unanodised state) cannot match.
How long to wait
Healing time depends entirely on the placement, not a fixed calendar date. A standard earlobe piercing is typically healed in six to eight weeks. Cartilage piercings such as helix, tragus, or conch commonly take four to twelve months. Ask the studio that performed your piercing when they consider it safe to change jewellery. What to Avoid
Sterling silver and gold-plated jewellery are not recommended for piercings at any stage. Silver tarnishes and can oxidise inside the piercing channel, and plating wears off with daily contact, exposing the base metal underneath, which is often nickel-heavy.
