Buying Jewellery for Skin That Reacts: A Metal Buyer's Guide
Somebody takes off a necklace and there is a red band across the back of their neck. Or earrings that felt fine for a week start to itch. We have this conversation at our counter most weeks, and the answer is usually simpler than people fear. It comes down to which metals sit in the alloy, and which parts of a piece touch skin the longest. This is a buying guide for the person doing the shopping.

What actually causes the reaction
Nickel is the first suspect and deserves to be. A 2019 clinical review in Contact Dermatitis states that nickel is the most frequent cause of contact allergy worldwide, and puts the prevalence in European adults at 8 to 19 percent, with a strong female predominance. The same review names jewellery and metal in clothing as the main causes.
North American figures point the same direction. A study of 44,097 patients patch tested by the North American Contact Dermatitis Group between 1994 and 2014 found an average nickel sensitivity frequency of 17.5 percent, rising from 14.3 percent in 1994 to 1996 up to 20.1 percent in 2013 to 2014. Jewellery was the single most common source of nickel contact. Those were people already sent to a clinic for skin problems, so the number describes that group rather than the general public.
Cobalt and chromium also sensitise skin, and gold itself can too. The mechanism explains our advice later on. Metal does not irritate skin by touching it. Sweat pulls metal ions out of the alloy, those ions pass into the skin, and the immune system responds. More sweat and more contact time mean more metal released.
What "hypoallergenic" means on a Canadian label
It means whatever the seller wants it to mean. Canada has no legal definition for the word as applied to jewellery, no test behind it, and no body checking the claim. A piece labelled hypoallergenic may be implant-grade titanium, and it may be plated base metal with a nickel core.
A Canadian buyer sits in a different position from a European one. The EU set a nickel release limit in Directive 94/27/EC of 30 June 1994: articles in direct and prolonged contact with skin must release less than 0.5 micrograms per square centimetre per week. Earrings, necklaces, bracelets, rings, watch cases and straps are all named. Coated pieces must hold that limit for at least two years of normal use.
Piercing posts got a tighter rule ten years later. Directive 2004/96/EC of 27 September 2004 set the limit for post assemblies inserted into pierced ears and other pierced body parts at less than 0.2 micrograms per square centimetre per week, because a healing piercing puts metal against broken skin.
Canada has nothing comparable for adults. The Children's Jewellery Regulations (SOR/2018-82) cap lead at 90 mg/kg and cadmium at 130 mg/kg in jewellery for children under 15. Nickel does not appear in the text. With no legal ceiling to lean on, the question you ask the seller does the work the law does in Europe: what is this made of?
The metals we recommend, and why
Implant-grade titanium is where we start for posts and studs. The two specifications you want on a receipt are ASTM F136, covering wrought Ti-6Al-4V ELI alloy for surgical implant applications, and ASTM F67 for unalloyed titanium. These exist because the material goes inside human bodies, which is a harder test than an earlobe. The 2023 MedComm review notes that titanium allergy has a low prevalence rate.
Niobium is the quieter option. It takes colour through anodising, which grows a coloured oxide layer on the metal itself rather than plating a second metal on top. Nothing flakes off because nothing was added.
Platinum works through simplicity. Most platinum jewellery is 950 parts per thousand platinum with iridium or ruthenium making up the rest, leaving little room for anything reactive.
High-karat gold follows the same logic. At 18K the piece is 75 percent gold by weight and 25 percent alloy. At 10K it is 41.7 percent gold and 58.3 percent alloy. White gold is the one that surprises people
Gold comes out of the ground yellow. Making it white means adding a metal that bleaches the colour, and nickel has done that job for decades because it whitens hard and costs little. Palladium is the alternative, and it costs more.
So a customer can wear a yellow gold chain for years, buy a white gold ring at the same karat, and react within a fortnight. Same gold content, different alloy. When someone describes that pattern, the whitening metal is the first place we look.
Palladium white gold removes the nickel and solves it for most people. One caveat is worth knowing in advance: palladium and nickel cross-react. The MedComm review reports mean palladium allergy at 7.8 percent among dermatitis patients, and palladium-only sensitivity at 0.2 percent. Almost everyone who reacts to palladium also reacts to nickel. For a confirmed nickel allergy where certainty matters, platinum or high-karat yellow gold avoids the question.
White gold is also finished with rhodium plating, a thin bright layer over the alloy that separates skin from the metal underneath while it holds. It wears through at the back of a ring shank in a year or two of daily wear, and that is when a quiet ring can start causing trouble. We wrote about what happens as rhodium wears off.
Sterling silver usually gets blamed unfairly
Sterling is 92.5 percent silver and 7.5 percent other metal, normally copper. Silver is a rare contact allergen and copper at that share seldom causes trouble. Two other explanations cover most cases we see.
First, the small parts. A sterling necklace can have a clasp, jump rings, or earring backs bought in as finished components, and those are not always sterling. A pendant stamped 925 tells you about the pendant.
Second, tarnish. Silver reacts with sulphur in the air and forms a dark film that transfers to skin, leaves a grey mark, and can irritate. Clean the piece properly and wear it again before concluding anything.
The parts that touch skin longest
This is the practical heart of it. A pendant swings free and spends its day against fabric. The clasp sits against the back of the neck, warm, catching sweat. An earring post sits inside a channel through the earlobe for sixteen hours. A ring shank wraps a finger with nowhere for moisture to go.
Contact time and sweat drive how much metal gets released, which is why the EU wrote a stricter limit for piercing posts. When you buy for someone with reactive skin, ask about those components by name rather than about the piece as a whole. How to shop, in order
Start by finding out what the person already wears without trouble. If they wear 18K yellow gold daily and react to one white gold ring, you have your answer without any testing. Then ask the seller for the alloy of every part: body, posts, backs, clasp. Let them wear the piece a few hours before committing to a full day, and check the skin underneath. Nickel dermatitis can take 24 to 48 hours to show, so one uneventful evening is not proof.
For pieces they already own and love, a clasp or a set of posts can often be replaced in titanium or gold at our bench for less than the cost of buying again. Clear nail polish is an evening's patch and wears through in days.
Last point, and we mean it. This is buying advice rather than medical advice. If someone's skin keeps reacting, or a rash spreads beyond where the metal sat, they should see a doctor or dermatologist. Patch testing is the only way to identify which metal is responsible, and knowing that turns guesswork into a short shopping list. Which metals are the lowest risk for reactive skin?
Four hold up well in practice. Implant-grade titanium, made to ASTM F136 or ASTM F67, is the one we point to most often for posts and studs. Niobium behaves in a similar way and takes colour through anodising without plating. Platinum is normally alloyed at 950 parts per thousand with iridium or ruthenium, which leaves little room for anything reactive. High-karat yellow gold works because 18K is 75 percent gold by weight, so the alloy portion is small. None of these is a guarantee for every person, and skin that keeps reacting deserves a doctor rather than a metal swap.
Does white gold contain nickel?
Often, yes, and this catches people out because they assume gold is gold. Gold is yellow, so making it white means adding a whitening metal, and nickel has been a common choice for decades because it whitens strongly and costs little. Palladium is the other route. A 14K nickel white gold ring is 58.3 percent gold by weight, and the remaining 41.7 percent is where nickel can sit. Yellow gold at the same karat usually uses copper and silver in that portion instead. If someone reacts to a white gold ring and wears yellow gold without trouble, the whitening metal is the first thing we look at. Ask for the alloy in writing before you buy. Is palladium white gold a safe swap for nickel white gold?
It removes the nickel, which is the main point, and for most people that solves the problem. One caveat is worth knowing before you spend the money. Palladium and nickel cross-react in patch testing, so a person already sensitised to nickel may respond to palladium as well. So a positive result does not always mean the ring on your hand is the cause. Does Canada limit nickel in jewellery the way Europe does?
No, and Canadian buyers should know this before they shop. If your skin reacts repeatedly, ask your doctor about patch testing, which is the only way to identify the specific metal responsible.
Does clear nail polish on a clasp actually work?
It buys you an evening, and we tell people that plainly rather than selling it as a fix. A coat of polish puts a physical barrier between the metal and your skin, and while it holds, release into the skin stops. The problem is that it wears through in days under a clasp that opens and closes and sits against a damp neck, and it fails without announcing itself. You find out when the rash returns. The EU rule on coated items asks that a coating keep the release rate down for at least two years of normal use, which is the standard a real solution meets. Nail polish is nowhere near it. For a piece you want to keep, replacing the clasp or the posts in titanium or gold is a permanent answer and costs less than most people expect.
